Fertility Grant Eligibility Criteria
To qualify for a grant from MJ’s Legacy Foundation, applicants must meet the following criteria:
Financial Need: Household income less than $250,000
Insurance Status: Be uninsured for fertility treatments or have exhausted infertility coverage.
Medical Evaluation: Be evaluated by, and under the care of, a Board Certified Reproductive Endocrinologist who has diagnosed the cause of infertility and has prescribed a treatment plan.
Location: Be a resident of Harris or Galveston Counties in the state of Texas
You may apply for a grant with any income level, but priority will be given to the applicants who:
Have a household income less than $150,000
Have primary infertility (no living children)
Are under 40 years old
Have not undergone previous IVF cycles
Please Note: We do not offer grants for oocyte (egg) or sperm cryopreservation, where the grant recipient has no immediate plans to attempt conception. This is due to a number of factors, including our limited funds and continued changes in technology and techniques surrounding retrieval and cryopreservation. This does not apply to applicants who are preserving eggs or sperm due to cancer or other medical diagnosis.
